Dear London Fire Commissioner (London Fire Brigade),

This request is now more than two weeks overdue please can you ensure a response is given.

Yours faithfully,

Tim Maidstone

London Fire Commissioner (London Fire Brigade)

Dear Tim,

Thank you for your Freedom of Information request, received on 15 April 2020, in relation to LFB forms.

I am very sorry for my delayed response, the forms you have requested are usually held in hard copy for use at incidents or at fire stations. Nevertheless, our Design team do hold PDF copies of the forms and they have sent these through to me to consider their release under the FOI act:

1) Form 2416 - Indemnity & Undertaking (pad) (F4460)
2) Form 18 - Vehicle Accident
3) Form 6178 - Control Information
4) Form No. 9: Fires or Special Service Calls (F4159)
5) Form 5683 - Message Pad
6) Form 5283 - London Fire Brigade and Police Service Deliberate or Fatal Fire Reporting and Recording Form (RRF1)
7) Form 6176 - Operational Equipment Incident Handover Sheet
8) Form 6182 - Key Decision Log
9) Form 6173 - Casualty Report Form

We do not typically provide copies of LFB forms under the FOI act as, if they are made public, there could be a risk of them being used in some element of criminal activity. Therefore this information is exempt from disclosure under the FOI act under section 31(1)(a) - Law enforcement. Section 31 is a qualified exemption and I must consider the public interests for and against disclosure. By disclosing this information there is a risk that it could make the LFB more vulnerable to crime. For example, it may be possible to print and use the LFB branded forms to impersonate a Firefighter which could, in turn, put other people at risk. I am therefore of the view that it is in the public interest not to disclose information at the detailed level you have requested.

I am sorry this that this may not be the answer you were hoping for but I trust you understand our position. Should you have any further questions please do let me know.

This request has been dealt with under the Freedom of Information Act 2000. If you are dissatisfied by this response you can request an internal review by writing to the Head of Information Management at the address below (or email [email address]).

Further information about your information rights (including how to raise a concern or make a complaint) is available from the Information Commissioner’s Office on their website at www.ico.org.uk or by writing to them at Wycliffe House, Water Lane, Wilmslow, Cheshire SK9 5AF.

Kind regards,

Zoe
