Dear NHS England,

Please direct this FOI request to NHS South, Central and West Commissioning Support Unit.

Dear CSWCSU,

Thank you very much for all your previous FOI responses.

I would like to make another request under the FOI Act.

For the purposes of the Act, please take the date of your receipt of this request as 8th January 2016.

• Please would you send me the agenda, all circulated items, attachments and documents, and the minutes (draft or approved) of any and all Hampshire Health Record Operational Group Meeting/Board Meeting and/or equivalent regular operational meetings held after or on the 1st March 2015 and before or on 31st December 2015.

These would include information regarding:

Information & IT Steering Group (IITSG) Meetings
Hampshire Health Records Programme Board Meetings
Hampshire Health Record Delivery Board Meetings
HHRA Updates for HHR Board Meetings

My request is not directed at any one particular CCG, but feel free to "nominate" NEHFCCG if you feel that is necessary to process my FOI request.

I would be grateful if you would be kind enough to send me the requested information promptly and in any event not later than the twentieth working day following the date of receipt of my request.

I would be grateful if you would kindly acknowledge receipt of this request as recommended by the ICO (“It would be good practice to acknowledge receipt of requests and to refer to the 20 working day time limit, so that applicants know their request is being dealt with”).

Thank you once again.

Kind regards,

Dr Neil Bhatia
