I note with interest an article (link below) about "Connected Care", another Graphnet shared record database, applying across Berkshire. It does appear to be identical to the Hampshire Health Record.

I would like some information about this project, which I shall refer to as "CC" for ease in this FOI request, if I may.

Please could you provide me with the following:

• Could you kindly tell me who the data controller is for the Connected Care shared record database (e.g. is it NHS SCWCSU?)

• Could you kindly tell me which GP surgeries, and in which CCGs, are uploading information from their records to CC

• Could you kindly tell me the names of the "6 local authorities" that are extracting and uploading information to CC (presumably social care records)

• Could you kindly tell me whether the presence of any of the read codes 9Nd1, 93C3, 9Ndo, 9Nu0 or 9Nu4 would automatically, and completely, prohibit extraction and uploading of personal confidential information to CC from GP records?
• If so, which read codes?

• Could you kindly tell me whether combined information held in the Connected Care database is being, or is planned to be, used for secondary purposes (such as risk stratification, healthcare planning, research or commissioning)?
• If so, please could you tell me precisely what the effect of the 9Nu0 read code (Type 1 objection) is on any secondary processing of uploaded data by Connected Care, if that objection is present in a non-dissenting patient's GP record

NHS England holds some information relating to this request. Some of the
information requested is not held by NHS England as recorded information
as the database is not yet live.

 

At present the contract with Graphnet has not been finalised so none of
the questions in the request can be answered in a definitive form.  At
this stage we can provide some information based on initial planning but
this is subject to change.

 

-   Could you kindly tell me who the data controller is for the Connected
Care shared record database (e.g. is it NHS SCWCSU?)

 

The Connected Care shared record database does not yet exist. At this
point in planning we believe that the appropriate model is likely to be
‘Data Controllers in Common’ with all organisations that may be sharing
data into the system in the future, sharing responsibility as data
controllers. We have an IG steering group (chaired by the LMC) providing
guidance in this and other similar matters.

 

-   Could you kindly tell me which GP surgeries, and in which CCGs, are
uploading information from their records to CC

 

The Connected Care shared record database does not yet exist – currently
no data is being uploaded. It is anticipated that, over time, all GP
practices will be uploading information.

The following CCGs are involved:

 

NHS Bracknell & Ascot Clinical Commissioning Group

NHS Newbury & District Clinical Commissioning Group
NHS North & West Reading Clinical Commissioning Group

NHS Slough Clinical Commissioning Group

NHS South Reading Clinical Commissioning Group

commissioning)?

 

The Connected Care shared record database does not yet exist – currently
no data is being uploaded. It is anticipated that secondary use of the
combined information, e.g. risk stratification (the identification of
locations or groups of patients who might benefit from improvements to
care delivery) will form part of the solution at some point in the future.
Any future secondary use will be agreed by the IG steering group and the
organisations providing the data.

*The IG steering group cited is used for the connected care programme
only. They were formed (July 2015) as an advisory group to the main
project Board.

 

-   If so, please could you tell me precisely what the effect of the 9Nu0

You may be interested in a number of FOI requests I have made, and are making, to GP surgeries with the heading, 'Data protection of health records', for example:

Some of these requests have received a response by separate email or have received no response. Only one practice, so far, has refused to provide information and will be referred to ICO.
All will, in due course, be followed up in one way or another. The only obstacle is finding the precious commodity of time!
