FREEDOM OF INFORMATION ACT 2000
FREEDOM OF INFORMATION (SCOTLAND) ACT 2002

In terms of the above legislation I would be most grateful if you
could answer the following:-

1] Please can you advise the TOTAL number of disciplinary HEARINGS
at the DWP for ANY and ALL computer misuse infractions (NOT just
social network access on work computers, but ANY computer
orientated disciplinary event) concerning staff at the DWP and
internet connectable computer equipment? The period to be covered
is from 1st September 2011 to 30th September 2011. Both days
inclusive? Thank you.

2] Please can you advise the TOTAL number of disciplinary WARNINGS
(verbal and written) at the DWP for ANY and ALL infractions (NOT
just social network access on work computers, but ANY computer
orientated disciplinary event) concerning staff at the DWP and
internet connectable computer equipment? The period to be covered
is from 1st September 2011 to 30th September 2011. Both days
inclusive? Thank you.

3] Please can you itemise the nature of each computer misuse as per

4] Are you aware that to date, there have been serious allegations
in the media that the DWP have some culpability in the DEATHS of
TWELVE innocent people including a 5 month old baby, and an unborn
child due to the problems with the current welfare reform systems?
These include cases such as Richard Sanderson who committed suicide
when the DWP withdrew (via their local authority DWP benefit
administering agency) Housing Benefit of £30 per week.

5] What measures are the DWP taking to fix their defective system
that has left innocent people like Mrs Sanderson without a husband,
or father to their child?

6] Was the £30 saving made by the DWP worth the life of this
talented, but uneployed aircraft pilot?

Quote-marks Police have confirmed a body found in the River Wear five weeks ago is that of the 30-year-old, who disappeared five months ago.

The sales coordinator, who had battled depression for a number of years, had taken a turn for the worse after receiving a letter telling her she had to be assessed by a doctor to see if she was fit to return to work.

REQUEST FOR INTERNAL FREEDOM OF INFORMATION REVIEW

Please pass this on to the person who conducts Freedom of
Information reviews.

I am writing to request an INTERNAL REVIEW of Department for Work
and Pensions handling of my FOI request 'FOI Request Concerning DWP
Computer Misuse'.

1a. I am disappointed at your refusal to answer my Freedom of
Information Request by apparently stonewalling it. I alleged your
grounds for refusal are far from solid, and against the spirit and
letter of the Freedom of Information Act (FOIA). Please supply a
copy of your public authority's statutory guidance under the FOIA
requirement for the DWP to...

“provide advice and assistance, so far as it would be reasonable to
expect the authority to do so, to persons who propose to make, or
have made requests for information.”

1b. Please confirm whether or not you have failed in this duty by
the evident obstructing of my reasonable request?

2. It would be helpful if you could substantiate your allegation
that my request is part of a 'campaign'? I am making an effort to
navigate your labryrinthal and tortuous bureaucratic system to
study whether your organisation has committed any computer misuse,
and the extent of that misuse? This is a serious matter.

3. My grounds at APPEALLING questions 1, 2 and 3 are that it is in
the PUBLIC INTEREST to assertain whether or not any computer misuse
has occurred at the DWP, or in concert by DWP employed agencies,
and if so to what extent that misuse is endemic?

FURTHERMORE you can expect subsequent requests of this nature. NOT
as vexatious, but in an effort to statistically and forensically
examine whether the DWP and it's agencies' computer misuse problem
is getting BETTER or WORSE? This again is in the PUBLIC INTEREST.
The PUBLIC INTEREST being whether the DWP are able, and be seen via
FOIA to be able to fix their defective computer misuse problem?

With regard to questions 4, 5 and 6, it is NOT within your LAWFUL
gift to decide whether to comment or not. The law is clear.
Particularly Section 1 of the Freedom of Information Act 2000. You
are REQUIRED to answer.

I APPEAL your negligence in failing to answer question 4. I shall
clarify and rephrase, removing the Data Protection Act detail you
are using as a shield to answer this legitimate and most grave
Freedom of Information Request.

REPHRASED QUESTION 4.....

"Are you, the DWP, aware that to date, there have been serious
allegations in the media and by HM Coroners, that the DWP have some
culpability in the DEATHS of TWELVE innocent people including a 5
month old baby, and an unborn child due to the problems with the
current welfare reform systems?"

SUPPLEMENTARY QUESTION 4b:

"Are you, the DWP, aware that if the DWP FAIL to address the
problem of aleged DWP WELFARE REFORM DEATHS, the Treasury and the
Taxpayer may be placed in harms way with exposure to multiple court
actions, civil and criminal, which will make the
pensions-misselling-scandal, and loan-insurance-misselling scandal
pale into insignificance?

SUPPLEMENTARY QUESTION 4c:

"Have you, the DWP, conducted proper due dilligence and risk
analysis at the potential cost to the Treasury and Taxpayer if and
when the legal profession awake to the fact that there are serious
defects within the DWP WELFARE REFORM SYSTEM which is leading to
allegations of DEATH and hazard to the health of the 11,000
disabled people and people on benefits being assessed each week by
the DWP and their agencies?"

REPHRASED QUESTION 5.....

I APPEAL your negligence in failing to answer question 5. I shall
clarify and rephrase, removing the Data Protection Act detail you
are using as a shield to answer this legitimate Freedom of
Information Request.

"What measures are the DWP taking to fix their defective system
that has left innocent people bereaved by the death of their loved
ones. To date 12 DWP WELFARE REFORM DEATHS?"

QUESTION 6.....

This can be removed from the request. It is rhetorical. You should
be thoroughly ashamed and alarmed that a husband and father has
died in order to save your organisation £30 per week. You SHOULD
also be taking immediate measures to ensure this does not happen
again. I will not be holding my breath for your Secretary of State
to attend to the matter of alleged multiple deaths in this Freedom
of Information process. He, and your department would be better
tasked to FIX this defective WELFARE REFORM SYSTEM that is causing
hazard to health and DEATH outwith the Freedom of Information Act
arena. Otherwise your next legal questions will be based on more
serious legislation than FOIA, with heavier penalties for ignorance
of the law, and the alleged operation of a dangerous and hazardous
DWP Welfare Reform system.

I am actually already aware of the list in fact I took the suicide report I posted as an annotation from the list, thank you for compiling it. I have been adding the links as annotations on this site in order to make people aware of the suicides.

It is my contribution to the Big Society, now that the Office For national Statistics are no longer compiling statistics on suicides and coroner are reaching a lot more narrative verdicts and therefore avoiding recording suicides as such.

Strange isn't it that these changes should come about at the same time as the Welfare Reforms and the potential associated rise in suicides as a result. Still that's where my big Society contribution will hopefully help.

Quote-marks Recently this was printed in a local paper Haverill Weekly News."Peter Duut, a Dutch national who lives in Haverhill, has worked long hours as a carpenter in the UK for the past two-and-a-half years.
When he became term-inally ill with cancer in April he tried to claim benefits, only to be told that he does not qualify.He and his wife had to live on 12P per day.
