Dear Charlie

Thank you for your information request dated and received by us on 30
April 2014

This request has been handled under the Freedom of Information Act 2000.

You asked us in the email below.

1. On how many occasion in the past 3 years has Jackie Nash attended a
cremation of foetal remains / still born infant on behalf of the Trust?

2. Is it standard practice for the bereavement councillor or a midwife
to attend cremations for foetal remains / still born infants? Please
provide the related policy or procedure is this is standard practice.

3. What are your procedures for burial of foetal remains including:

Are individual burials provided?

Are these individual burial plots?

Are burials plots an 'adult' size plot divided up?

Are permanent memorials allowed?

Our response:

Question 1,

No  -Jackie Nash has not attended any cremation or burial on behalf of
the trust.

It is not standard practice for the Bereavement midwife or any other
midwife to attend a cremation or burial and therefore it is not in a
policy, however many midwives do this to support the families that they
know and have cared for over many months out of concern, care and
kindness and where the family have specifically asked them if they would
like to attend.

Question 2,

No, this is not standard practice

Question 3,

Foetal remains over 14 weeks gestation are buried  (or cremated)
individually, which means they are in their own individual coffins.

These are not individual burial plots.

There are two special plots dedicated for the burial of foetal remains.

Permanent memorials are  Not allowed
