Dear Imperial College Healthcare NHS Trust,

On how many occasion in the past 3 years has Jackie Nash attended a cremation of foetal remains on behalf of the Trust?

Is it standard practice for the bereavement councillor to attend cremations for foetal remains and still born infants? Please provide the related policy or procedure is this is standard practice.

What are your procedures for burial of foetal remains including:
Are individual burials provided?
Are these individual burial plots?
Are burials plots an 'adult' size plot divided up?
Are permanent memorials allowed?

Yours faithfully,

Charlie Milton
