Dear The Healthcare Safety Investigation Branch,

Matt Hancock was unable to provide this data on television when asked for same.

Please provide asap all recorded information and documentation re fatalities from Covid 19 vaccines -Pfizer Moderna and Astrazeneca wirhin 28 days of vaccine being taken.Please advise age per fatality and name of vaccine per fatality and age per fatality.

The HSIB do not collect this data so I'm sorry we cannot respond to your questions.
