Dear Northern Health and Social Care Trust (Northern Ireland),
I am requesting information as per the following:
According to the performance report Sept 2013 there was a reported 935 Falls reported in the hospital setting.
I am requesting:
1-A copy of the quarterly reports on falls for Jan-Mar 2013 & Apl-Jun 2013 and Jul-Sept 2013
if these reports do not contain the hospital and ward that these falls occurred in I am also requesting in a separate format a list of each fall that should include: The date of the fall, the hospital it occurred in and the ward/area within the hospital.
2- In each instant shown separate state if the patient suffered injury from the fall
3- in each instant shown separate how many investigations took place if the patient died after the fall
