If forms were placed on their website it would save people having to queue more than once at tube ticket offices and reduce inconvenience when Oyster cards stop working!

Dear Mr Rawlins

Thank you for your email to the Oyster Card Helpdesk. I'm sorry for the
delay in replying to you.

In order for me to help you please provide me with more information.
Please send me your full Oyster card number and security answer (mother's
maiden name, memorable place or date). Please also let me know what type
of Oyster card you have. If this is an adult Oyster card, and your Oyster
card is not function correctly you can simply visit any Tube station with
a proof of ID and address and get this replaced by filling in a failed
Oyster card form. The Tube station will then provide you with a new Oyster
card, containing the same balance and/or Travelcards as your old one.

Alternatively you can post your Oyster card to us with a cover letter
including the above reference number and your full postal address and we
will be able to refund any remaining pay as you go and/or deposit by
cheque. If you choose to do so please post this with a short cover letter

If you send this card to us you are able to get a new Oyster card from any
Tube station ticket office, alternatively you can purchase a card either
over the phone by calling 0845 330 9876 or online by visiting
