Dear Department for Work and Pensions,

Please send the information you hold on failed interactions

The stages and process and authorisation and decision making that lead to sanctions then closure

Including the safeguarding in place to protect vulnerable customers, or complex needs customers, or have lcwra, or are mentally ill /mentally incapable , those who struggle to maintain a claim, communicate and interact with DWP and claim management and reviews.

Also safeguarding those who identified as needing advance customer support and those with disability act 2010 needing reasonable adjustment such as telephone, writing or home visits who failed interactions, failed to make contact, failed to respond, failed appointment.

Including the protection and safeguarding in place to prevent and stop sanctions or claim closure when a customer is at risk of harm or risk of death if sanctioned or claim closed. And whom and how the process making the decision that it is a risk, and options for what happens for example must continue to keep attempting contact.
