can you please send me a copy of your reply to an FOI about 3 years
worth of bonuses and expenses for staff/ceo and executive
directors, that was sent around the end of November to NHS Trusts.

How much was paid (a) in total to all staff, (b) to the chairman, (c) to
non-executive directors, (d) to the chief executive and (e) to executive
directors in bonuses/non-consolidated performance pay awards in each of
the last three financial year for which figures are available

The information requested can be found in the following documents for
2008/09 and 2007/08.

The information relating to 2009/10 is exempt under Section 22 of the
Freedom of Information Act 2000, as this document is due for future
publication during September 2010.

How much was re-claimed (a) in total for all staff, (b) by the chairman,
(c) by non-executive directors, (d) by the chief executive and (e) by
executive directors for personal travel expenses for (i) mileage, (ii)
parking, (iii) taxis, (iv) car allowances, (v) trains, and (vi) flights in
each of the last three financial year for which figures are available

I can confirm that from the information requested we are able to provide
the following in attachment 1:

(c) Non-executive Directors

(d) Chief Executive

(e) Executive Directors

The information that you have requested in sections (a) and (b) is not
currently readily available as we are in the process of changing to a new
expenses system. Therefore to extract this information from the current
system it would involve a manual search which would take this section of
the request outside of the cost limit of **450.

Information relating to 2006/7 would need to be searched for and collated
manually due to the fact that North Stoke and South Stoke Primary Care
Trusts merged during October 2006 and were using separate systems. This
manual search will fall outside of the cost limit of **450 and is
therefore exempt under Section 12 of the Freedom of Information Act 2000.

How many official vehicles are currently allocated to staff; what vehicles
are they, are any of the vehicles chauffeur-driven; which staff have
access to these vehicles; and at what cost have these vehicles been
provided at in each of the last three financial year for which figures are
available.

This information can be found within the annual reports of the
organisation (relating to directors on the VRM scheme). The links for
these reports are as above (question 1). To extract this information for
all staff would take this section of the request outside of the cost limit
of **450 as it will involve a manual search.

executive directors for (i) over-night accommodation (ii) hospitality
(iii) business phone calls (iv) meals (v) any other personal expenses in
each of the last three financial year for which figures are available

This question falls outside of the cost limit of **450, due to the fact
that requirement is to break down costs into subsections which do not
relate to those used for working purposed by the NHS and would necessitate
manual searches of individual personal files.

For any of the questions that have fallen outside of the cost limit, you
are able, if you so wish to either refine the question and bring it below
the **450 cost limit or alternatively request a fees notice which we will
happily provide.

thank you for your reply from the 22 June 2010 from my original FOI request dated 25/05/2010.

I was interested in the 2008/9 bonus payments to Directors as one figure and the CEO bonus as another.
I have looked at your 2008/9 Annual Report and Accounts, and although you have a total renumeration figure, I cannot see a breakdown giving the bonus element.
1) Could you therefore tell me whet these figures are?

2) Do you have Gifts, Hospitality and Sponsorship Policy or similar and a Code of Conduct covering this area and can you please email me a copy.
Can you send me a copy of your Gifts, Hospitality and Sponsorship register for 2008/9 showing details of individual entries.
If you wish, do exclude individuals details, but please include company details as it enables public scrutiny of the frequency of sponsorship.

I would be grateful if you assist and advise me about the information available to do with switchovers from Mixtard 30, eg do the Community Diabetic Specialist Nurses hold this information, or do GP practices?

No, Medicines Management did not monitor which insulin that patients were

the months before the withdrawal in Dec.

The DoH published an appraisal form under:

Are your Consultants required to complete Point "6. Probity" or
similar given in the appraisal document?

review. During the reviews no probity were issued.

I have a further question about Insulin medication, and various Mixtard 30 discontinuations.

Does the PCT hold information showing what the total PCT expenditure was for the following formulations in the periods:
a) April 2009 – March 2010,
b) January – February 2011

and for the total expenditure of these groupings:
i) Mixtard 30 in its various forms: Penfill 3ml, Innolet pre-filled pen device, 10 ml vial
ii) Humulin M3 Kwik Pen
iii) Humulin M3 Vial
iv) Humalog 25 in its various forms: 3ml cartridges, pre loaded Kwik pen, 10ml vial

I am not sure how long it takes for all expenditure in February 2011 to be recorded. If not all expenditure is available on receipt of this request, please do not supply the information for Jan, Feb 2011. If the Trust can then say when this information will be available, that would be helpful.

Additionally, if clarification is needed around the descriptions of the drugs, I would be grateful if the Trust can advise and assist me.
