Ex-employer will not provide P45

Dear Advisory, Conciliation and Arbitration Service,

I left my previous agency on the 26 September 2016, I came back to UK on the 21st November 2016. In the meantime I tried to get a job, I finally got one in December 2016, and approximately on the 20th of February 2017 I went to my previous agency to ask about my P45. They gave me a form to fill in and they said they would send it to HMRC and I will receive my P45 in maximum 4 weeks. Months have passed and I did not receive anything, I kept calling to ask about what has happened but I could not get a clear answer, only that there were delays, and only on the 12th July 2017 they said they would resend the P45 form to HMRC because the first employer of the agency never sent it the first time.
My question is, who do I call to make an official complaint about my previous agency, to get my P45 and the pay for my remaining holiday days?

I am sorry but the FOI Act only covers requests for already recorded
information in our storage systems not individualised advice on employment
problems. You will need to contact our employment advice line on 0300 123
