ESA Exempt Work (Permitted Work) forms.

Dear Department for Work and Pensions,

With the ending of the 52 week limit for those doing Exempt Work (Permitted Work) in ESA WRAG the PW1 form has been revised to version 4/17.

Presumably various other PW# forms have also been revised or withdrawn.

I am thinking in particular about the forms: PW6, PW7, PW8, PW9, PW11, & PW12, but also any others that have been revised or withdrawn.

Please could you inform me which of these PW# forms have been withdrawn, which forms have been revised, and which forms have remained the same.

Please also provide PDF examples of any of the revised forms, or weblinks where they can be downloaded.
