Your request has been handled under the Freedom of Information Act 2000 (FOIA).
DHSC does not hold the information you have requested.
However, we have additional information which we are providing to you on a discretionary
basis. This is outside the scope of the FOIA, and not subject to internal review.
The data collection forms are statutory forms, and DHSC is required to publish them.
However, all updates and changes are made by the National Child Mortality Database
team, who do so in conjunction with relevant experts in each area. You may wish to

The National Child Mortality Database is an NHS England funded programme which is
commissioned through the independent non-profit organisation HQIP (Health Quality
Improvement Partnership). You may therefore wish to contact NHS England about any
