Dear Child Maintenance Service,

Please can you confirm the following information for the years 2013, 2014, 2015, 2016 and YTD 2017:

In relations to companies in which a paying parent with a CMS open case (ie. the "employer"):

- Fines or sanctions for lack of the provision of information when requested
- Fines or sanctions for not applying a DEO when requested to do so
- Number of times the CMS has issued civil proceedings against an employer for refusal to comply with demands made by the CMS.

Where you cite sanctions please break these down (by type of sanction) where information is available.
