Dear Northern Ireland Ambulance Service Health and Social Care Trust,

Could I please make a request for information on the NIAS emergency ambulance control (EAC) centre telephone system crashes, specifically:

1. A list/log of occurrences for when the emergency call responding system based in Northern Ireland has crashed during the past two years (from 2013 to the most recent), to include as much as is possible of the following:
a) the date and time from and until for the duration of the crash
b) how many calls were affected, including the category of calls (i.e. A.B.C.) and the nature of the medical complaints involved during each crash
c) if and where the calls were diverted to and how they were subsequently responded to during each crash
d) the length of time in responding to calls (when paramedics arrived at scene) during each crash
e) the eventual outcome or review of the system due to each crash occurrence.

2. NIAS procedures and policies for dealing with such incidents.

I would prefer that the information in #1 be provided in tabular format such as CSV or Excel files.

However, there has been a delay with your request and if possible could this be extended for another 10 working days and a response will be issued to you on or before 3 November 2015.
