Thank you for your email of 28 September 2012) about practical driving
test records.

 

Before we are able to respond, I would like some clarification of the
information you have requested.  Please could you clarify the following:

 

1.  Would you like to know what driver details are held electronically
purely by the driving test centre or by DSA as a whole?

  

Please note that without clarification of your requirements we will be
unable to process your request.  If you do not respond within three months
from the date of this letter, then we will consider your request closed.

If you would like to make a request for your own personal information,
under the Data Protection Act 1998; you can do this by writing to the
address above; or alternatively you can email

If you would like to make a subject access request you will need to
specify where the information that you require, may be held.  For example,
a specific driving test centre.  In order to prevent disclosure of
personal data to third parties, we would also require some form of
identification.  Acceptable identification would include the photo page of
your passport, or if this is not available, the following; a copy of your
provisional or full driving licence and birth certificate - issued within
6 weeks of birth. We may also require proof of residence which could be
provided in the form of an original utility bill such as a council tax,
gas, electricity, water, telephone bill or bank statement.
