Question 1:         Does the trust have a strategy to manage patient
records electronically? If so please detail

Response:           The Trust can confirm that it is BS10008 Compliant.

BS10008 is the British Standard that outlines best practice for the
implementation and operation of electronic information management systems,
including the storage and transfer of information.  BS10008 outlines best
practice for transferring electronic information between systems and
migrating paper records to digital files.  It also gives guidelines for
managing the availability and accessibility of any records that could be
required as legal evidence.

 

Question 2:         Does the trust have an EDRMS (electronic document
records management system)?  If so which EDRMS software does the trust
have?

Response:           Evolve (provided by Kainos)

 

Question 3:         Which EPR System does the trust have?

Response:           Quadramed CPR

 

Question 4:         Does the trust have plans to procure an
EPR/EDRMS/Records management solution?

Response:           N/A

 

Question 5:         How many physical patient record sets does the trust
have?

Response:           Half a million physical patient records for current
patients, more at external storage site.

 

Question 6:         Does the trust utilise a 3^rd party storage
organisation to store /  manage pater notes?  If so please detail.

Response:           DataSpace.

 

Question 7:         Has the trust scanned any patient records – in-house
via a 3^rd party scanning bureau?  If so how many records have been
digitised?

Response:           Patient records have been scanned in-house via an
internal scanning bureau.  Approx 21% scanned based on table below.

Response:           47.54 WTE, please not that also included in this
figure are staff who undertake the Admissions function, whereby they
activate patients admission for both planned and emergency on the EPR.

 

Question 10:      What percentage of health record availability does the
trust achieve?

Response:           99.6% - Information from dashboard.
