Dear Southport and Ormskirk Hospital NHS Trust,

I’d be grateful if you could please advise:

1. If the Trust currently has an electronic order communications
system, ie a laboratory test ordering system in place?
a. If so, which system?
b. What was the cost of the system at the time of purchase?
c. In which hospitals and in which departments is the system
currently in use?
d. Has it been implemented for both wards and GPs?
e. Does the Trust have a current vendor contract for the software?
f. If so, when does it expire?
g. Does the Trust have a current support contract for the software?
h. If so, when does it expire?

2. If the Trust currently also has an electronic Admission,
Discharge and Transfer (ADT) system in place?

Thanks very much in advance.

Yours faithfully,

Andrew O'Hara
