have some case notes in the library around 4000, We see roughly 3000
patients a week

in Question 12 you state the records are stored in-house, whereas question 14 states they are stored off-site. If there is a provision for off-site storage could you please confirm who the provider is and when the current contract is due for renewal?

Question 14- How many files/boxes are stored off-site 15. If physical
files are  stored on-site, roughly how many files/boxes are in the trusts
library?

 

The Response for this question should be "We have no notes stored off
site". Sorry for the confusion. Hope this helps.
