ECOs errors in evaluating visa applications

Dear Home Office,

I would appreciate your answers my questions below:
1. What rules or procedures are in place to prevent cases where Caseworkers or ECOs disregard/overlook information already included in Visa applications?

2. Are there any procedures in place to ensure that the issue of ECOs/Caseworkers overlooking or disregarding vital information on applications does not repeat itself at all in the same case/application?

3. Is there any process i.e training and testing process in place to ensure that all ECOs adequately understand every part of the Tier 4 rules and do not misapply the rules? How often is this evaluated?

I ask the above questions because
my first refusal was as a result of an oversight by the first ECO who reviewed my application and I'm shocked that my second refusal which voids the reason for the first refusal is still as a result of oversight by the reviewing ECO. For the second time, important information on my application has been disregarded by the ECO(s).
These unjustifiable errors by the ECOs are disheartening and shocking even as I await the result of my current administrative review request which I submitted last night in the hope that the error will be rectified so that I can enrol on my course which commences soon.
