DWP Notifications from HM Coroners - Prevention Future Deaths

With regard to the protocol for HM Coroners to 'Prevent Future Deaths' (PFD) and their letters/reports as defined by paragraphs 28 and 29 of the Coroners (Investigations) Regulations 2013 and the Coroners Rules 1984, Section 43, plus any other similar legislative notifications aimed at preventing future deaths. Please can the Department of Work & Pensions (DWP) advise how many of these coronial reports to prevent future deaths following the death of a DWP client have been received by your department for:-

1] Calendar year 2010 - 1st January 2010 to 31st December 2010. For example the late Stephen Carré, where HM Coroner Mr Tom Osborne issued a Rule 43 report/letter as a result of your department's treatment of people with disabilities?

2] Calendar year 2011 - 1st January 2011 to 31st December 2011.

3] Calendar year 2012 - 1st January 2012 to 31st December 2012.

4] Calendar year 2013 - 1st January 2013 to 31st December 2013.

5] Calendar year 2014 - 1st January 2014 to 31st December 2014.

6] Calendar year 2015 - 1st January 2015 to 31st December 2015.

7] Current year 2016 - 1st January 2016 to 31st July 2016.

Please can the DWP name each coroner; each jurisdiction/coroner's court, and each of the deceased where public domain rules allow this (and if not, then appropriately redacted documents in lieu).
