RE: SUBJECT ACCESS REQUEST
I am writing to formally make a Subject Access Request for evidence of information that you hold about me to which I am entitled under the General Data Protection Regulation and the Data Protection Act 2018. You can identify my records using the following information:

Please supply the data about me that I am entitled to under data protection law including:

1. confirmation that you are processing my personal data;
2. a copy of my personal data;
3. the purposes of your processing;
4. the categories of personal data concerned;
5. the recipients or categories of recipient you disclose my personal data to;
6. your retention period for storing my personal data or, where this is not possible, your criteria for determining how long you will store it;
7. confirmation of the existence of my right to request rectification, erasure or restriction or to object to such processing;
8. confirmation of my right to lodge a complaint with the ICO or another supervisory authority;
9. information about the source of the data, where it was not obtained directly from me;
10. the existence of any automated decision-making (including profiling);
11. the safeguards you provide if you transfer my personal data to a third country or international organisation; and
12. any and all other information you hold about me and how it is used, stored and shared.

You issued a PCN against me for a bus lane contravention (MC36611341), which I appealed against and now I find that you have passed my details on to an enforcement agency by the name of Jacobs. I certainly did not provide you with the authority to provide any of my personal details to them. May I ask why you chose to do this?

If you need any more data from me, or a fee, please let me know as soon as possible. I can be contacted at the phone number and address at the end of this letter.

I look forward to receiving your response to this request for data within one calendar month, per the General Data Protection Regulation and the Data Protection Act 2018. If you do not normally deal with these requests, please pass this letter to your Data Protection Officer, or relevant staff member.

Thank you for your request received on 21 May 2021.
As you are seeking personal information, I would be grateful if you could
email the Council at [MCC request email] about your
request using a personal email address rather than via a public website
such as whatdotheyknow.com.
Please note that requests for information that relate to wholly personal
information about yourself fall within the scope of the General Data
Protection Regulation . This gives individuals access rights to personal
information held about them and operates alongside the Freedom of
Information Act, which aims to extend these statutory rights to cover
other information.
I would also recommend contacting whatdotheyknow.com and asking them to
remove this as this is your personal data and not a freedom of information
request.
The Council will also need to see evidence of your identity.
Please provide copies of at least two proofs of identity showing your
current residential address (not more than 3 months old).  The types of
documents the Council usually accepts to satisfy this requirement is
copies of recent Council Tax Bills, Utility Bills, Driving Licence or
similar document.
