Dear Cardiff and Vale University Local Health Board,

With regards to the process of medicine management of drugs.

Could you please provide the structure, positions and names for the medicine management team across the LHB?

Can you please explain the process of the application and how the process is affected by AWMSG/NICE guidance/recommendations?

How a consultant (in hospitals) or a GP (in primary care) can access a non-formulary drug for a patient and what process they would need to go through.

Yours faithfully,

David Smith
