I want to ask some questions below regarding your organisation’s
link with Dr Kate Mansfield, Wellington Road Family Practice Yate

1. What is the nature of the relationship between your organisation
and Dr Mansfield? (i.e. positions)

2. How did she get chosen for such relationship and/or position(s)?

3. Is she being paid for such relationship and/or position(s)? If
yes, how much and how frequent?

4. If the answer to question 3 is affirmative, please provide a
copy of Dr Mansfield’s contract with your organisation

5. Please provide a summary of what Dr Mansfield is supposed to
provide for your organisation

6. Please provide explanation whether Dr Mansfield has
satisfactorily completed each duty listed under question 5 in the
current financial year. If not, provide explanation.

7. Please provide a list of organisations that Dr Mansfield have
been in contact with to fulfil her duties under question 5.

8. Has Dr Mansfield had any previous criminal convictions?

9. Has Dr Mansfield had any previous incidents preventing her from
working with vulnerable adults, young people and children?

10. Has Dr Mansfield been subjects to complaints from patients?

11. Has Dr Mansfield undergone a vetting process to fulfil her
duties under question 5? If yes, please provide details and copies
of the vetting documents. If not, please explain why.
