Dear Northern Health and Social Care Trust (Northern Ireland),
I am requesting information as per the following:
1: a copy of the final death rate report carried out by Dr Foster
2: how much this review cost, supply all invoices.
3: provide all the raw data used by Dr Foster to be able to finish the report.
4: A copy of the model used by Dr Foster to include the model used to compare the observed and expected deaths
5: for the RAMI death rates currently used by the NHSCT RAMI (risk adjusted mortality index)provide for the values for the trust plus the observed deaths, expected deaths and Confidence Intervals.
