In these minutes, i quote pages 5 and 6 -

"The Committee received a report from Sam Foster, Chief Nursing Officer, Oxford University Hospitals Foundation Trust (OUH) on the current position of maternity services.

Members raised a number of questions:

"If the CQC’s ‘Musts’ have been implemented.”

Sam Foster responded as follows:
“The audits around the ‘Musts’ have given assurance that those improvements were in place.”

I also note, bottom of page 6
"Action...
Sam Foster to provide more detail on the CQC Action Plan Update and on the Lotus Team as well as the Maternity Safe Staffing Paper."

I would like to request copies of the Audits referred to on pages 5 and 6, and copies of docuemnts relating to the action plan or plans, and "action plan update" mentioned, as above.

I note that there is an item "That OUH offered to share more detail on the underpinning action plans arising from the CQC inspection", and an image of a PDF file "Briefing paper for
CQC action plan... " in the action tracker log of the next (JULY 2022) JHOSC meeting on the OCC website (see item 12 at the URL below) , but the embedded PDF is not possible to open.

This may be because the action tracker was originally a Word file containing embedded files, and has been flattened to a simple PDF.
That file would be a good start in answering this FOIA request, but is unlikely to contain all the relevant info. It would be sensible to produce the underlying quality assurance process documents and audits.

I am particularly concerned to receive all info related to audit and quality assurance regarding IEA 5 "risk assessment throughout pregnancy"; this was one of the Trust's 7 immediate and essential actions (IEAs) identified in light of the Ockenden report. (described in the Trust's Powerpoint which is also available with the MAY 2022 JHOSC papers).

If still in post, it may be that Sam Foster , and/or Matt Akid (mentioned in the JHOSC action tracker), and/or the OUH Chief Assurance Officer, and/or Dawn Gilkes, Senior Accreditation and Regulation Manager, can locate the relevant papers for you.
(Dawn Gilkes is mentioned for example in paper 11 "11 TB2021.70 CQC Maternity Report (pdf, 195 KB)" at the trust Board meeting 8th September 2021 -
