Documentation Used to gain Permission for Use of Individual Patient Information Passed to Google

Can you please inform me what documentation is used to record individual's permission for use of their information passed to Google (previously Google Deep Mind) Are people given a copy of the record stating permission?

That is incorrect. I am not referring to a specific research but the data UCL has a partnership with google whereby individual medical information is shared 'with permissions'.
