Document "Train and Station Services for Disabled Passengers: A Code of Practice"

Dear Department for Transport,

The DfT / Transport Scotland document "Accessible Train Station Design for Disabled People: A Code of Practice" dated November 2011 at (since superseded) states:

"The original version of this document, entitled Train and Station Services for Disabled Passengers: A Code of Practice, was published by the Strategic Rail Authority in 2002 and was known as the Blue Book. Its purpose was to fulfil the Secretary of State’s responsibility under Section 71B of the Railways Act 1993 (as amended), to produce a Code of Practice “protecting the interests of users of railway passenger services or station services who are disabled”."

Please supply copies of:
1) the "original document" as produced by the SRA in 2002
2) subsequent versions of the document, under their various names. The 2011 and 2015 versions are available online, but the 2011 version refers also to editions created in 2008 and 2010; and I don't know if there have been other versions.

If you could supply this I would be very grateful.
