Dear Department for Work and Pensions,
Fiona Shaw 13/11/1986 JT813060A – DLA ceased September 2019
As Fiona’s husband, I am assisting her along with citizen’s advice with her tribunal appeal as she has been refused Personal independence payment and at mandatory reconsideration the decision was upheld that she was awarded 0 points for daily living and mobility under their criteria.
We have appealed this decision and have now received notice of a tribunal dated 3rd April 2020 but require further medical advice to support our appeal.
Please can you advise if you are able to provide the medical information relating to the decision to award Fiona a lifetime award of DLA?
A copy of Fiona’s decision case report for her lifetime award or any submitted medical information would be helpful in providing supporting evidence detailing Fiona’s specific support needs relating to her epilepsy, migraines and specific learning difficulties
I appreciate any assistance you can provide in helping support our appeal – please call if further information required on 07828 798 452.
