This request was originally made to South Staffordshire Health Informatics Service who rather than provide an answer to my query told me a request would only be accepted and a response provided if the request was made to yourselves. While I note this is expressly not in line with the Freedom of Information Act, and a response should be provided irrespective of to whom the request is made, I have nevertheless passed the request on to yourselves.

As the original request was made on 7 September 2012 I expect a response within 20 working days of the original date of the request (ie, by 5 October 2012).
