Disclosure of reported paying parent suicides
Dear Child Maintenance Service,
Making reference to the FoI fullfilled under ref FOI2021/24889, a document was disclosed as an attachment with a footer dated 03/02/17. The document can be found here https://www.whatdotheyknow.com/request/7... and on page 2 of this document it states
"Notification of Death Reported
Where notification is received of a client suicide this must be immediately escalated to the Line of Business Director."
As this clearly states policy for reported suicides to be escalated to the Line of Business Director, please provide the following information:
1. The name of the Line of Business Director/s to whom reported suicides are escalated to (if there are multiple then please provide the dates during which each held this responsibility).
2. A monthly count of all male paying parent suicides reported from the date of this document (03/02/17) up to the latest available records.
3. A monthly count of all female paying parent suicides reported as per 2 above.
4. The name of the person to whom the Line of Business Director/s that you list report to and their position.
5. The frequency with which the Line of Business Director/s report to their superior/s regarding paying parent suicides reported to the CMS.
