Dear Nottingham University Hospitals NHS Trust,

Can you provide in writing and by use of diagrams the formal disciplinary structure from used from first instance to dismissal for all employees please.

I would like to know how many disciplinary hearings were made involving doctors, junior doctors and consultants in 2015, 2016 and 2017 to date. Please provide a list of all such hearings and indicate what action was taken if any. I realise that names may have to be redacted, but you should show nature of case and whether it involved a fatality or not, number of people being held up for possible disciplinary action, and whether the case was closed and whether the outcome was to take no action, issue a warning/caution, or if discipline was taken, nature of that discipline.

You will receive the information requested within 20 working days allowed
under the Act unless the Trust does not hold the information or there is a
reason for it to be withheld. I will write to you in any event.

 

If you have any requirements regarding the format any information should
be supplied in, e.g. the language to be used, audio, large print and so
on, then please let me know.
