Please can you provide me with the following details:

By each department, for the years 2013/2014 (to date), 2012/2013, 2011/2012, 2010/2011:

How many employees of the council were subject to disciplinary action?

How many disciplinary cases of employees of the council resulted in the employee being suspended pending the outcome of the subsequent investigation?

What is the basis (reason) of the suspension ?

How long did each suspension last?

What was the outcome of the investigation for the employee ie no case to answer to, a verbal warning, a first written warning (for how long would this remain on file), a second written warning (for how long would this remain on file), a third written warning (for how long would this remain on file), or dismissal. I require the basis of the suspension to be ‘married up’ with the length of each suspension, and the outcome of the investigation, to make the information meaningful.

Whose decision is it to take disciplinary action, and whose decision to suspend?

If an employee is dismissed by the council, what is the policy in terms of their being able to be re-employed by the council in the future?

I would like the information to be provided in a table format.

The relevant Council Service(s) has searched for the requested information

Please note that information has been anonymised to prevent individual
employees from being identified. In addition to this, figures of less than
3 employees have been removed and replaced with an indication to this
