Discharged and then readmitted

Dear Wye Valley NHS Trust,

I would like to know how many people that have an operation at Hereford County Hospital, who are then discharged (either to home, or a community hospital )but then return back to the hospital within 1 week of first being discharged.

I would like to know from which ward they were discharged from originally.

I'd like to know this for 2018 and 2019 so far.
