Diabetes Services in Hywel Dda
Dear Hywel Dda Local Health Board,
I understand since the abolishment of the LDSAGs, Diabetes Planning
and Delivery Groups (DPDGs) were to be set-up to lead, manage and
report on improvements in diabetes services. These DPDGs will put
in place delivery plans for the NSF Standards and will lead and
manage the implementation of the Plan.
Could you therefore please provide me with:
1. The members of the LPDGs within this LHB
2. Any published plans they have agreed on.
3. Any plans for diabetes service delivery within the newly formed Network community groups.
One further request if possible is a list of practices in the LHB
that have taken up any diabetes enhanced services (DES).

Information Requested Under The Freedom Of Information Act 2000
Thank you for your recent request for the supply of information relating
to Local Diabetes Planning and Delivery Groups.
Under the Act, the Health Board is required to supply the information to
you within 20 working days. In terms of your request, this means that the
information should be provided by 8^th December 2011. If this is not
possible, a further letter will be sent advising you of the progress made
in satisfying your request.
Please find attached our leaflet giving guidance on our procedure for
managing requests for information that is covered under the Freedom of
Information Act 2000.

Hywel Dda Health Board Diabetes Action Plan. As this plan is draft and
awaiting ratification in Spring 2012 we are unable to supply you with the
document at this stage. Please contact us again next year to check on its
progress.
§ A list of practices in the LHB that have taken up any diabetes

§ These are attached as a spreadsheet.
