While Mr Chambers was credited with producing the Chief Executive Officer's Reports for March and May 2023, I observed that by July, this responsibility had shifted to Abigail Jago. There seems to be no clear communication about Mr Chambers's departure, the circumstances surrounding it, or an official announcement of Abigail Jago's assumption of the Interim Chief Executive role. To this end, I would appreciate answers to the following enquiries:

1. Please can you specify the commencement and conclusion dates of Tony Chambers's tenure as the Interim Chief Executive for the Queen Victoria Hospital NHS Foundation Trust?

2. Please can you shed light on the reasons behind Mr Chambers's departure from the Interim Chief Executive position?

3. Please can you detail the compensation package, including any perks or reimbursements, that Mr Chambers received during his term as Interim Chief Executive Officer?

4. Are there any ongoing or concluded investigations concerning Mr Chambers, especially pertaining to his function as Interim Chief Executive Officer or any matters of child or vulnerable adult safeguarding?

I am writing to request an update on my Freedom of Information (FOI) request, which I submitted to Queen Victoria Hospital NHS Foundation Trust on August 18, 2023.

Under the Freedom of Information Act 2000, public authorities are required to respond to FOI requests within 20 working days, unless there are exceptional circumstances. It has now been 22 working days since I submitted my request, and I have not yet received a response.

I am concerned that the response to my request is delayed. I would appreciate it if you could provide me with an update on the status of my request and an estimated date when I can expect to receive a response.

If I do not receive a response to my request within the next 5 working days, I will be forced to refer my complaint to the Information Commissioner's Office.

I would like to remind you that the Information Commissioner has the power to investigate complaints about FOI requests and to take action against public authorities that fail to comply with the Act.

I hope that we can resolve this matter without having to resort to further action.

Please can you specify the commencement and conclusion dates of Tony
Chambers's tenure as the Interim Chief Executive for the Queen Victoria
Hospital NHS Foundation Trust?

Mr Chambers’ tenure at QVH commenced on 1 February 2023 and concluded on
30 June 2023.

Please can you shed light on the reasons behind Mr Chambers's departure
from the Interim Chief Executive position?

Mr Chambers’ appointment was on an interim basis, and he resigned on15 May
2023, providing notice to the end of June. For reasons of confidentiality
we are unable to provide further details about Mr Chambers’ departure;
however we can confirm that it was not linked to any patient safety or
quality of care concerns.

Please can you detail the compensation package, including any perks or
reimbursements, that Mr Chambers received during his term as Interim Chief
Executive Officer?

Mr Chambers was paid a salary of £200,000 per annum pro rata. He was paid
no benefits and claimed no expenses. Mr Chambers did not actively
contribute to his NHS pension scheme while at QVH, and QVH has made no
contributions to his pension.

Are there any ongoing or concluded investigations concerning Mr Chambers,
especially pertaining to his function as Interim Chief Executive Officer
or any matters of child or vulnerable adult safeguarding?

There are no ongoing or concluded investigations into Mr Chambers, and no
whistleblowing concerns or complaints have been raised about him in
relation to patient safety or quality of care.
