Dear Department for Work and Pensions,

In VTR 3739-3312 you said 'Every medical assessment is then audited until the HCP produces four consecutively that are classed as being of “A” grade standard.'

Please can you tell me:

1) What are all the grades?

2) Are the grades percentage based?

3) If so, list percentage required for each grade.

4) If not then how is each grade calculated? Please fully describe the process on determining the grade.

5) Please fully describe the criteria of the audit.

6) Who (not an individual) performs the audit?

7) What safeguards are in place to prevent abuse of the audit process?
