Subject: RE: breach of confidentiality at DORSET HEALTHWATCH
Good afternoon Mr Crow
Thank you for getting back in touch.
Healthwatch England is not responsible for taking on escalated complaints made about local Healthwatch services, such as Healthwatch Dorset. It is instead the responsibility of the local authority to take them on, as long as you have attempted to formally complain to the local Healthwatch service in the first instance.
We have contacted Bournemouth, Christchurch and Poole council (BCP) regarding this matter. BCP have given us a contact email address to pass onto you so that you can raise the issue directly with them - [email address].
Kind regards
Mark

Dear Wayne,
Thank you for sending over the additional information and documents. A senior medico-legal adviser has since reviewed these, and has the following suggestions, beyond what you have already tried.
There appears to have been a breach of confidentiality issue with your local Healthwatch, please see further information here from the Citizen’s Advice Bureau regarding breach of confidence issues –
Yours faithfully,
Wayne Crow

Dear NHS Dorset, FOI Requests (NHS Dorset),
I will happily publish the previous request and forward a copy via the royal mail due to the images contained
However may I point out that the CCG were provided with extra copies as was Michael Tomlinson MP, so please contact him as he states in his letter which the PHSO has held since August 1st 2019 he is aware of the issues with the NHS, social services and police
The fact that the NHS chief executive officer acknowledged the receipt of the freedom of information request which included proof of delivery of freedom of information requests made to the police and social services but yet failed to uphold the legal and moral responsibilities of the public service act does not inspire confidence
Nor does the conduct and response I have recently been forced to endure
As such please verify the address you wish a copy of the outstanding freedom of information request to be sent, as I will happily send it recorded delivery and subsequently publish the proof of postage and delivery
As NHS Dorset have proven multiple times they cannot be trusted
Yours sincerely,
Wayne Crow
Dear Requester,
We apologise for the confusion that this has caused, we were not aware
that your current request was linked to a previous request which would
have been made to the NHS Dorset Clinical Commissioning Group (CCG). This
will have been filed separately with the CCG FOI requests, as our
organisation is now known as NHS Dorset Integrated Care Board (ICB) and
has a different Chief Executive. The reason we are asking for the
reference number that was on the CCG response to you, is so that we can
hopefully quickly locate the documentation relating to your original
request without inconveniencing you any further. The reference number
should be found at the top of the CCG response to you. Alternatively, if
you have the date of your initial FOI, we may be able to search using that
rather than you having to post a copy of this to us. The NHS retention
period for FOI requests and responses is 3 years, and as long as your
original request was made within the last 3 years, we should still hold
this information. If it is outside this time frame we will require a copy
of the full request from you so we can investigate further, which can be
sent to:

Dear NHS Dorset, FOI Requests (NHS Dorset),
Thank for your response and excuses, however please note a further request was made last year after the refusal of insulin
At which time the IBC was requested for the audio recording of my telephone contact with the IBC to be sent to the CQC and EASS
Please contact social services for verification of this
Note I have received a apology from 111 for the refusal of insulin, however the NHS has not explained why 111 did not have access to records of my essential medication hence the pharmacist having to inform 111 of medication and dosage
Hence the ongoing case numbers below
Insulin: Dorset PCC reference number - 2023-24-034 .. Shore medical (Parkstone towers) CQC ref: CAS-166016-K4K1C9 .....Department of health ref: DE-1481240
Note how the CQC have also been advised of public health Dorset response to the continued abuse of human rights and medical ethics
Yours sincerely,
Wayne Crow
Dear NHS Dorset, FOI Requests (NHS Dorset),
Further to your email please note the letter from NHS resolutions was regarding the letter from PALS which was included in documents provided to the parliamentary health ombudsman acknowledging the declaration document
And the subsequent claim this letter did not exist
Not the multiple freedom of information requests which the NHS England chief executive acknowledged
And the freedom of information request which was acknowledged was provided to the local MP who was aware of the local government ombudsman case which had gone unanswered, a copy of the LGO case document has been supplied to the PCC and social services and I am still waiting on there responses
However as the authorities in Dorset have no interest in the public interest only political interests I do not expect them to respond honestly but rather evade there obligations
The following is the contents of the declaration document
Which the NHS have previously acknowledged
Mr w a Crow
23 Sydling Close

Dorset bh17 8sy
NHS number: 412.599.3378
Ref: Parliamentary Health Onwardsman C2078289
Ref: Parliamentary local government Onwardsman 170045505
To whom it may concern
With deepest regret is must ask the medical personnel present to sign the following declaration.
I believe this action necessary due to the failure of the NHS trust and CCG in Dorset to uphold the standards, policy and procedures of the NHS set nationally including the Patients Charter
I am also aware that No action has been taken following the formal complaint regarding the actions of Miners-Wallis reported in September 2018 at my last diabetes review and the statements of the GP Aizpitarte , and the lack of a appropriate response is a direct contravention of the General Medical Council standards regarding, Professional and Ethical conduct within the medical profession
These failures I believe clearly demonstrate “ A Complete Betrayal of the principals of the Medical Profession and the NHS “ and show contempt by senior health service personnel for the hard work of frontline staff and the patients
Declaration
I .................................................. will uphold the National standards of the NHS and the general medical council with regard the treatment of the patient Mr Wayne Crow, and the treatment recommended is for his medical needs and not for the benefit or at the request of 3rd or 4th parties
I will also maintain confidentiality and act in a professional and ethical manor towards the patient at all times
Sincerely
W A Crow,
Yours sincerely,
Wayne Crow

Dear NHS Dorset, FOI Requests (NHS Dorset),
With regards information you will be able to reference
On the 20th June 2023 I received a letter from the Dorset diabetic eye screening programme (DESP)
Regarding the cancellation of my appointment due this process there was email communication which included resending the original defferment request, following the confirmed breech of data protection at HealthWatch Dorset which allowed the NHS to produce a fraudulent responce to a formal complaint
The text below comes from the deferment document, as provided to the Dorset PCC and CQC, along with BCP council and councillors

Dear sirs,
Unfortunately due to the continued failure of the authorities in Dorset to fulfill there obligations and deliver services to expected standard, I believe it is necessary to defer my scheduled diabetes eye appointment
I hope the information contained below further demonstrates my reasons
Plz find
Screenshot of email communications, which took place during the time period the phso state the NHS took no action and NHS claim don’t exist
Email from NHS southwest stating my request not to be harresed by Cranford heath group practice
Email from IPCC stating Dorset police are to fulfill there obligations and investigate the crime report made after receiving the Phso findings in December 2020
Please note this decision to defer has not been taken on a *whim* as I believe the betrayal of frontline personal who have earnt my trust and respect is important
Good afternoon Mr Crow,
Following our conversation on the telephone this afternoon. I have noted that you are having difficulty with access to your email and using your tablets. Please could you acknowledge that you have received this email?
Also if you could please return the consent form as requested.
If there are further issues you wish to raise that have arisen since September 2018 – please include these in your reply. I will then open a new reference for you and endeavour to gain a response to your concerns.

Thank you for your response however I am now more confused as the letter shown below and a PALs response state they have no record of the complaint
Please note the referenced visit in the attached email refers to my delivery of the PHSO finding in which they refer to the aledged fraudulent responce from the NHS and how the NHS failed to respond to the formal complaint made
Please note how the receipt for the documents delivered was forwarded to the CQC
Dear Mr Crow
I am emailing following your visit to Sentinel House on Friday 15th January 2021 and the emails that we received from you during the course of last week.
Firstly, I am sorry that your complaints about NHS and social care services continue to cause you distress and I do hope that you are able to resolve your concerns and restore your trust and faith in the NHS.
I need to explain to you that Sentinel House is not currently open to members of the public because of the COVID-19 (Coronavirus) pandemic. Furthermore, the current government guidance is that you must stay at home in order to protect the NHS and save lives. This is the law, and the Police can take action against you if you leave home without a reasonable excuse, and issue you with a fine (Fixed Penalty Notice). If you visit Sentinel House again and manage to gain entry to the reception area, you will be asked to leave.
With regards to your complaints about health and social care services, you first contacted us in November 2018 and said you wanted to make a complaint; however, you did not respond to our request for information about your complaint. Therefore, we did not receive a complaint from you at that time regarding the contents of a consultant psychiatrist’s letter. Complaints about NHS services should be made within 12 months of an incident or of the matter coming to your attention and, for this reason, we are not able to now investigate a complaint about something that happened in 2018.
From the information you have provided to us, you have raised your concerns about the Canford Heath Group Practice with a number of NHS organisations, the Parliamentary and Health Service Ombudsman, the Local Authority, and the Police. Dorset HealthCare does not provide GP or social care services and we are therefore unable to investigate concerns about these services. We also cannot investigate alleged criminal misconduct; this is a matter for the Police. I am aware that Dave Way, Dorset HealthCare’s Data Protection Officer and Information Governance Manager, emailed you on 22nd December 2020 about this.
We will not be able to respond to further correspondence that we receive from you unless it relates to Dorset HealthCare services and falls within the usual timescales that I have outlined above.
I trust this email clarifies the situation. In the meantime, if you do not wish to contact your GP practise for clinical advice please contact the NHS 111 service on telephone 111. You can find further information on the NHS website.
Yours sincerely

Dear NHS Dorset, FOI Requests (NHS Dorset),
Thank you for your response however I am now more confused as the letter shown below and a PALs response state they have no record of the complaint
