The format of the Conditional Survey Report from Derek Hammond display below that Iain Turner has provided as Conditional Survey as coerce a course of Health and Safety Risk and Hazards under the Decent Home Act to the Tenant Ewan Natty THE FOI REQUESTER.
For a Dangerous Building under the Building act of 1984. As this Conditional Survey Report is a FAKE and it is NOT recognised as a LEGAL CONDTITIONAL SURVEY REPORT.

I’m sorry you are unable to read the document in the format provided. I sent it as a PDF which is generally viewable on most smartphones, tablets or computers. If you open it using a PDF application such as Adobe it should open. If you would like it in alternative format then please let me know which type and I will do my best to provide it to you.
I’m afraid that we do not have a signed copy of the report. The data is entered into our asset management system and the surveyor’s name recorded against it. The report provided is an export of the data in our system. We are not required to keep an original signed copy. There is also no requirement for a stock condition surveyor to be MRICS qualified.
