Derbyshire Vascular Network - Submitted Annual Self-Assessments

Dear NHS England,

Recently you supplied me with information regarding the vascular services specification Ref: FOI-060054, l would be grateful if you could now supply me with copies of the last submitted annual self-assessments made by Chesterfield Royal Hospital Foundation Trust and the University Hospitals of Derby and Burton NHS Foundation Trust (that together form the Derbyshire Vascular Network).

I do not know the name of the self-assessments, but they are the self-assessments submitted to you in respect to the Specialised Vascular Services (Adults) specification No: 170004/S. The self-assessments as advised by you will contain information as to what documentation has or has not been produced, you gave me an example of an Indicator Code: 170004S-001 / Indicator Name: There is an agreement outlining the network configuration.

The information l am seeking regarding the Derbyshire Vascular Network made up of the aforementioned trusts is a list containing all of the documentation relevant to the Specialised Vascular Services (Adults) specification No: 170004/S that has or has not been produced as stated in the last submitted annual self-assessments along with the relevant indicator codes.

I am hoping this is a simple matter of just supplying me with electronic copies of the self-assessments submitted to you, to negate yourselves from spending time having to produce a list from them etc.

If for whatever reason the full self-assessment fails any sort of public exemption test and/or contains personal details of people and as such cannot be released, then l would be grateful if you could just supply a list of all the indicator codes/names regarding documentation that has or has not been produced as stated in the last submitted annual self-assessments regarding the documentation as of the Specialised Vascular Services (Adults) specification No: 170004/S.

I have assumed that both of the aforementioned trusts that operate the Derbyshire Vascular Network are commissioned by you to do so.

NHS England holds some this information.

Please find attached, as a PDF document, the Quality Surveillance
Programme self-declaration for vascular services at the Royal Derby
Hospital (part of the University Hospitals of Derby and Burton NHS
Foundation Trust) submitted by the Trust on 30 June 2019.

There is no equivalent report for the Chesterfield Royal Hospital
Foundation Trust. This is because self-declarations are required only of
those centres that are ‘hubs’ within vascular networks and not from those
centres that are spokes. Chesterfield is a spoke in the Derbyshire
network, and does not undertake arterial surgery. When such surgery is
required for Chesterfield patients it is carried out in Derby.

Thank you for your response regarding FOI - 1910-1082631, the information is is exactly what l was looking for, l do however just ask for one simple clarification regarding your response.

The Derbyshire Vascular Network (as l have been advised by the hospitals) is made up of both Chesterfield Royal Hospital NHS Foundation Trust and the Royal Derby Hospital (University Hospitals of Derby and Burton), l assumed (rightly or wrongly) that self-assessments regarding the Specialised Vascular Services (Adults) specification No: 170004/S would be submitted by both of theses hospitals (and as such l requested both) but you have only supplied the one, completed by Derby Royal Hospital.

Please can you clarify why you have only sent the one self-assessment, for example is it because Chesterfield is not a part of the Network as l have been advised (i.e. not commissioned by you to provide Specialised Vascular Services) or is that that only the main arterial centre completes self-assessments or is it just missing from your response by mistake etc?
