1. How many DoLS assessments for standard authorisation have been conducted by Herefordshire Council staff between 1st January 2017 and 28th February 2018, for individuals in residential care homes, whether in Herefordshire or another county?

2. Of the DoLS assessments for standard authorisation conducted by Herefordshire Council staff between 1st January 2017 and 28th February 2018, how many were granted?

3. Of the DoLS assessments for standard authorisation conducted by Herefordshire Council staff between 1st January 2017 and 28th February 2018 how many were not granted?

4. Of the DoLS standard authorisation granted, how many form 5 documents were signed by the current director for Adults and Wellbeing (Mr Martin Samuels)?

5. Of the number of DoLS standard authorisation granted form 5 documents signed by the current Director for Adults and Wellbeing (Mr Martin Samuels) how many form 3 and form 4 documents content were checked for factual accuracy or truthfulness?

6. Of the number of DoLS standard authorisation granted form 5 documents signed by the current Director for Adults and Wellbeing (Mr Martin Samuels) how many form 3 and form 4 documents content were not checked for factual accuracy or truthfulness; please note I am aware of at least three such occurrences?

7. Of the number of DoLS standard authorisation assessment documents, how many had their content challenged for accuracy or truthfulness by a third party?
i.e. a third party being an attorney, deputy, interested party or RPR etc.?
Please provide a separate count for each DoLS form, ie. form 3, 4,5 etc.

8. How many cases have occurred where the deprivation of liberty authorisation has been appealed in the Court of Protection by Herefordshire Council?

9. How many cases have occurred where the deprivation of liberty authorisation has been appealed in the Court of Protection by a third party?
E.g. a third party being an attorney, deputy, interested party or RPR etc.?

10. How many cases, if any, did the Court overturn the deprivation of liberty authorisation?

11. How many cases, if any, did the Court of Protection place further conditions on the deprivation of liberty authorisation?

12. Separately, for standard applications, please indicate which authorisations related to people whose care was funded by the local authority and those where funding was provided by other sources, i.e. health care funding, commissioned or privately, etc.?

Q4a. How many individuals within Herefordshire Council are in the pool of people who are authorisers or signatories of Deprivation of Liberty authorisations, both standard and urgent?
Please provide the number of authorisers or signatories between 1st January 2017 and 28th February 2018.

Q9a. How many cases have occurred where the deprivation of liberty authorisation has been appealed in the Court of Protection by any party between 1st January 2017 and 28th February 2018?

Q9b. How many Section 21a appeals to The Court of Protection occurred between 1st January 2017 and 28th February 2018, regarding vulnerable individuals subject to a DoLS authorisation by Herefordshire Council between 1st January 2017 and 28th February 2018?

Q11a. How many cases, if any, did the Court of Protection place further conditions on the deprivation of liberty authorisation of any person between 1st January 2017 and 28th February 2018?
