Delayed publication of Fatal Incident Reports

Dear Prisons and Probation Ombudsman for England and Wales,

I was shocked to discover today that quietly, without explanation, you casually slipped into the public domain without any explanatory Press Release the drug related death of a man in HM Prison Durham that happened on 9th January 2011 - 10 years and one month ago.

1, What is your policy, in terms of time scale and other actions, for publication of completed Fatal Incident Reports?

2. Once an Inquest has been completed what is your policy, in terms of time scale and other actions, for publication of Fatal Incident reports?

3. How many other completed Fatal Incident Reports are still awaiting publication where the death occurred
a. in or before 2010
b. in 2011
c. in 2012
d. in 2013
e. in 2014
f. in 2015
g. in 2016; and
h. in 2017
