Delayed Discharges of Older Persons

We're waiting for Naeem Ahmad to read a recent response and update the status.

Dear Royal Cornwall Hospitals NHS Trust,

I hope you are well.

I appreciate there are several questions below, however I am trying to understand this important and costly issue and feel this information will help an organisation perhaps help the Trust at a future date.

1. For the period 1st April 2013 – 31st January 2015, what is the average length of time for an older person (+65),
(once considered medically fit) to be discharged into:

a) Nursing Home
b) Community Hospital
c) Residential Home
d) Home (with a package of care)
e) Home or family member (without any package of care)

If the above is not available, then perhaps use your categories of:

i) Simple – Self-care
ii) Simple – Return (to care home)
iii) Simple – Restart +/-
iv) Simple – Reablement (including new POC, STEPS and Community Hospital referrals)
v) Complex (inc. Fast Track)

2. How much does this average discharge time cost the Trust? The costs queried relate to the total cost of the wait of the above delay from decision to discharge, then that figure broken down on an average of per person for the period 1st April 2013 – 31st January 2015

3. What is the average cost per night of stay (as an inpatient medically fit
waiting for discharge), for an elderly person at RCH Treliske, West Cornwall Hospital (WCH) and the St Micahel’s Hospital (SMH) within the Trust for the period 1st April 2013 – 31st January 2015?

4. What is the average number of operations cancelled due to the above delays in discharge for the period 1st April 2013 – 31st January 2015?

5. What is the average cost of a cancelled operation at RCH Treliske, WCH and SMH for the period 1st April 2013 – 31st January 2015?

6. What is the average cost associated with delays in cancelled operations, (e.g. lost potential income) due to delayed discharges of an older person at RCHT, WCH and SMH for the period 1st April 2013 – 31st January 2015?

7. What is the average cost of a bed at RCH Treliske per night per person for the period 1st April 2013 – 31st January 2015?

8. What is the average cost of a bed at West Cornwall Hospital per night per person for the period 1st April 2013 – 31st January 2015?

11. How many Care Home residents (older persons), including Nursing, Residential and EMI were admitted on an emergency basis in the last 2 years?

12. What number of beds were occupied, on average, by an older person for unplanned admissions at RCH Treliske, WCH and St Michaels Hospital for the period 1st April 2013 – 31st January 2015?

13. What was the average number of beds occupied by an older person for planned operations for the period 1st April 2013 – 31st January 2015?

14. Has the Trust calculated the cost of timely discharges of the elderly verses delayed discharges?

15. Has the Trust identified the causes of the delays in discharges of older persons, in any internal reviews or audits etc? Has it formulated a strategy in response to the causes of delay?

16. Is the Trust compensated by Social Services if a delay in discharge of an older person is caused by Adult Social Care, Pennisula Community Health or similar partner organisations?

17. What is the total number of older persons discharges from the Discharge Lounge at RCH Treliske? Based on the categories in question 1 (either a-e or i-v), what were their destinations?

18. How many older persons have died whilst waiting to be discharged? How many of those who died were a delayed discharge?

If you do not hold the information for the dates shown, we could perhaps discuss alternative time sets so the compiled information is comparable.

Further to my email correspondence below, please now find the
clarification you requested pertaining to our response of Questions 5) and

* Question 5) - The cost of a cancelled operation is contractually
agreed between the RCHT and its commissioners as a local price
arrangement.
* Question 7) - The RCHT holds a database which calculates the cost of a
patient's episode of care with the Trust.  It calculates a cost per
bed day, per ward stay but we unable to break this down further.
