What are the Trust's policies and procedures and relevant accountabilities - particularly with respect to communicating with the patient - when the Trust decides not to make a appointment that has been previously been prescribed by a specialist, who (at the time) appropriately informed the patient and noted the required appointment on the patient's record?

Thank you for your response to my enquiry of 23rd May 2014, further to which I shall be grateful for your confirmation that:

when the Trust decides not to make an appointment that has been previously prescribed by a specialist, who (at the time) appropriately informed the patient and noted the required appointment on the patient's record,

then, at the time the decision is made not to make the appointment, the patient should be appropriately informed and offered an alternative appointment.
