Please could I just clarify your response please. When the number of deaths is requested you have said 'see below'. Is the answer that there has only been one death of a person with a learning disability in the last three years, and that it occurred in 2017-18 and was reported on NRLS and STEIS?

There have been approximately 26 Learning Disability deaths in the Trust, the death that was reported to NRLS was reported as there was a Datix completed for that episode of care, the other deaths would be reviewed as normal under our Mortality Review Process.

We only started the Leder process in conjunction with the CCG at the end of November 2017 so the recorded information we have is as follows:

Wef Jan - Dec 2018 there were 17 deaths
Wef Jan 2019 - 1/4/2019 there were 9 deaths
A total of 26 deaths.
