1) the number of deaths of patients with a learning disability that occurred in the last three years (by years defined as 1/4/16 - 31/3/17, 1/4/17 - 31/3/18, 1/4/18 to present).

2) the number of deaths of patients with a learning disability reported as an incident on the National Reporting and Learning System (NLRS) in the last three years (defined as above)

3) the number of deaths of patients with a learning disability reported as an incident on the Strategic Executive Information System (STEIS) in the last three years (defined as above)

Under Section 1(1)(a) of the Freedom of Information Act 2000, I can
confirm that The Shrewsbury and Telford Hospital NHS Trust holds
information that falls within the definition in your request which is set
out below:

 

1)         the number of deaths of patients with a learning disability
that occurred in the last three years (by years defined as 1/4/16 -
31/3/17, 1/4/17 - 31/3/18, 1/4/18 to present).

The deaths of patients with Learning Disabilities were not separately
identified from the general population prior to 1st April 2017.

2017-18 = 16 deaths

2018-19  year to date = 9 deaths

 

2)         the number of deaths of patients with a learning disability
reported as an incident on the National Reporting and Learning System
(NLRS) in the last three years (defined as above)

2 deaths

 

3)         the number of deaths of patients with a learning disability
reported as an incident on the Strategic Executive Information System
(STEIS) in the last three years (defined as above)

2 deaths

 

 

Please also find attached a briefing which explains your rights to request
a review by the Trust or to complain to the Information Commissioner’s
Office if you are not satisfied with the response from the Trust, and an
outline of the Trust’s policy on the Reuse of Public Sector Information.
