Sorry for the delay in replying to your email. Unfortunately the COT and Food Standards Agency do not have the information you require. However you may want to contact the Office of National Statistics (https://www.ons.gov.uk/) as they have statistics on death rates attributable to different causes, although it is unclear whether they would hold information on caffeine specifically. Otherwise the National Poisons Information Centre (http://www.npis.org/) may have better information.

If you are not satisfied with the way your request for information has been handled, you should write within two calendar months of the date of this letter to the FSA’s Openness Team, and ask for an internal review. They will arrange for the FSA’s Complaints Coordinator to conduct the review. If you are not content with the outcome of the internal review, you may apply directly to the Information Commissioner for a decision. Generally, the ICO cannot make a decision unless you have exhausted the complaints procedure provided by the Agency.
