All of the ONS mortality data comes from the information collected at
death registration. All of the conditions mentioned on the death
certificate are coded using the International Classification of Diseases,
Tenth Revision (ICD-10). From all of these causes an underlying cause of
death is selected using ICD-10 coding rules. The underlying cause of death
is defined by WHO as:
a) the disease or injury that initiated the train of events directly
leading to death, or
b) the circumstances of the accident or violence that produced the fatal
injury

The table below shows the number of deaths at York Hospital by underlying
cause of death, for 2001 to 2009 (the latest year available). The causes
of death have been grouped according to ICD 10 chapters, and also include
several common causes of death such as lung cancer and ischaemic heart
disease.

Please bear in mind that the place of death recorded at death registration
cannot always be reliably translated into a specific, known healthcare
organisation or site. The registrar of births and deaths is required by
law to record the name and address of the place where the death occurred,
and this information is provided by the qualified informant (usually a
relative of the deceased). If a death is referred to the coroner and an
inquest takes place, the place of death comes directly from the coroner
via a 'Form 99 (REV)'.

Special extracts and tabulations of mortality data are available on
request from the mortality team (email [email address]). Please note
that special data extracts may incur a charge.

I hope the data is useful and suitable for your needs.

You have the right to have this response to your freedom of information
