Dear Ms Rushbrooke

Thank you for your email. As advised in the response, NHS Wandsworth CCG
does not hold all of the information you have requested, and is unable to
supply some of the information requested. The CCG does not hold conclusive
data on all residents and only holds information for the time period
between January 2014 and September 2015.

From the data which is held by the CCG, I can confirm the following
information:

·        Number of residents at York Court care home who died between
January 2014 and September 2015 = 14

With regards to the number of residents who suffered pressure ulcers, the
CCG cannot provide an exact figure due to restrictions regarding the
release of personal information that relates to living individuals.
Disclosure of this information could potentially lead to the
identification of individuals as the numbers are very low.

·        Number of residents who suffered pressure ulcers between January
2014 and September 2015 = between 1 and 5

I hope this clarifies the CCG’s response.

Kind regards

Kimberley

Dear FOI (NHS SOUTH EAST COMMISSIONING SUPPORT UNIT),
Many thanks
Please advise what action is taken/the process after an ‘external’ Serious Incident
is reported by the Trust on the national STEIS reporting system that the CCG does have access?
Whose responsibility it to report the serious injury to the CQC?
and ensure a safeguarding is referred if required?
and that a RCA and SIF is undertaken ?
How can a copy of the STEIS report be obtained ?
and where does that report ultimately end up?
who ensures and monitors the process re grade 3 pressure ulcers is completed and undertaken?

Yours sincerely,

Dear FOI (NHS SOUTH EAST COMMISSIONING SUPPORT UNIT),

Thank you
As per the response as follows;
The CCG understands that serious incident notifications on the STEIS system are not exempt from the Freedom of Information Act 2000 as a category and therefore public bodies would be expected to disclose information they hold on serious incident notifications, although in order to comply with the Data Protection Act 1998 and the common law duty of confidence it would be necessary for any content that could potentially identify an individual to be redacted.
*and where does that report ultimately end up?
The STEIS system is accessible to the CCG which is the main commissioner of each NHS Trust, and that CCG can access notifications reported by that provider. NHS England has access to the STEIS system and designated staff can access all notifications.

In view of the above please can I be provided with all steis reports relating to York Court care home Battersea between Dec 2014 and Dec 2015

It has become clear since 2014 that the care home owned by FSHC York Court in Battersea that FSHC closed Dec 2015 has dark secrets witheld by all - the abuse and negelct that went on in this care home from 2014 and the at least 56 complaints logged with the CQC and 37 safeguardoings - has resulted in all involved witholding information regarding that home that is in the pubkic interest and relatives interest and the suffering of multiple residents concealed - all resposonsible but no one made responsible or accountable for the known abuse by staff who worked there - that ranged from bullying, theft, abuse and neglect acts of omission and ignored alerts - the MCA 2005 ignored as was NHS legal rights - the constant refusal by all those involved to divulge information that probably amounts to evidence of the scale of the problem is indicative of the failing care of our loved ones who suffer silently
